Job Profile Summary

This role focuses on safeguarding and promoting the welfare of patients and can provide services if necessary. In addition, this role focuses on performing the following Social Work duties: Provides social work assistance to patients and families relating to illness, disease, hospitalization and life events in all areas of service as a member of multi-disciplinary team and in liaison with community resources. Responsibilities also include identification, assessment, and coordination of cases and anticipation of needs related to illness, hospitalization, finances, education, and home/community service. A professional individual contributor role that may direct the work of other lower level professionals or manage processes and programs. The majority of time is spent overseeing the design, implementation or delivery of processes, programs and policies using specialized knowledge and skills typically acquired through advanced education. An experienced level role that applies practical knowledge of job area typically obtained through advanced education and work experience. Works independently with general supervision, problems faced are difficult but typically not complex, and may influence others within the job area through explanation of facts, policies and practices.

Job Overview

This position will work with the Social Work Department to link the medical center with community resources, prioritizing overseeing the Guardianship process. Additionally, this position may provide supervision and oversight of resource coordination, office administration and IT support.

Job Description

Minimum Qualifications:

1. Bachelor’s degree or equivalent.

2. One (1) year of experience with Social Work Services.

Preferred Qualifications:

1. Bilingual.

Duties and Responsibilities
: The duties and responsibilities listed below are intended to describe the general nature of work and are not intended to be an all-inclusive list. Other duties and responsibilities may be assigned.

1. Works with medical team to identify patients and families who require assistance with obtaining resources to enter rehabilitation facilities. Most specifically, Guardianship applications.

2. Works directly with Social Workers, the Medical team and Risk Management.

3. Assists the team to help educate patients and families on guardianship, prepare paperwork, work with outside counsel to organize paperwork, attend court sessions with family.

4. Delivers resources to patients and families in the form of coordinating transportation, completing paperwork for services, helping patients navigate complexities of community resources.

5. Maintains collaborative team relationships with peers and colleagues in order to effectively contribute to the working group’s achievement of goals, and to help foster a positive work environment.

6. Ensuresevidence-based, best practices and standards of care are clinically practiced.

7. Provides consultation in the delivery of care that inspires excellence in knowledge, skill, and leadership at the clinic level.

8. May oversee office administrative staff, office management.

9. Encourages staff to pursue growth and development in their role as resource specialists.

10. Facilitates continuous performance feedback with staff in order to advance their knowledge, skills and practice.

11. Directly responsible for onboarding new staff, ensuring up to date licensure and credentialing.

Physical Requirements:

3. Considerable reaching, stooping, bending, kneeling, crouching.

4. Frequent exposure to hazardous chemicals, sick patients, bodily substances, noise and possible exposure to radiation, lasers, electric shock, etc.

5. Regularly exposed to the risk of blood borne diseases and other transmissible infections.

6. Contact with patients under wide variety of circumstances.

7. Subject to varying and unpredictable situations.

8. Handle emergency and crisis situations.

9. Subjected to irregular hours.

10. May have contact with hazardous materials.

Skills & Abilities:

1. Excellent interpersonal and organizational skills.

2. Able to work with detailed, confidential material.

3. Computer experience.

4. Familiarity with a hospital setting.

5. Ability to read/write and communicate in English.

6. Ability to function well in very busy situations.

7. Responsible and reliable.

8. Good organization skills.
